Healthy teeth in the mouth, where digestion begins, mean healthy digestion. The teeth lost by people have been tried to be replaced with prostheses. However, prostheses made when implants were not commonly used cause different problems for patients regarding both eating and speaking. With the use of implants, these problems were overcome. With the use of implants, chewing and speaking functions were improved.
All on four method has been applied since 2003. It is a treatment method based on fixing prostheses in patients with excessive bone loss. Successful results have been obtained in patients who cannot use prostheses due to bone loss with the procedure.
All on four is an implant treatment applied to patients with missing all teeth. All on four provide patients with comfort as if they had natural teeth. All on four is a procedure consisting of a prosthesis fixed on four implants placed in the jaw.
The difference between the All on four technique from standard implant techniques is that the posterior implants are placed at different angles, and the expected 3-6 month recovery period without teeth is eliminated. It also allows patients with bone loss in the posterior tooth region to use fixed teeth.
What is All-on-Four?
Oral and dental health is now one factor that determines people's quality of life. This is also effective in the planning of dental prostheses. Patient satisfaction is desired to be increased with the procedures made.
In cases of complete edentulism seen in patients, it may not be possible to place implants in these areas as a result of cases such as jaw bone loss, reduction of the distance between the nerve canal in the lower jaw, and sinus sagging in the upper jaw. Advanced surgical procedures are required to apply implants to these areas. This brings both a 1-year treatment and an economical cost.
In traditional implant methods, it is tried to obtain bone with bone graft in cases where the jaw bone is insufficient. These procedures both prolong the treatment period and leave the patient toothless in this process.
With All on four, the patient is allowed to have a fixed prosthesis in the middle of these problems. The patient gets implants in the morning and temporary fixed prostheses in the evening. After an average recovery period of 3 months, permanent prostheses are attached, and the patient does not have a toothless recovery period. The fixed prosthesis obtained with All on four makes chewing more effective, painless, and trouble-free. In addition, bone tissue is preserved by preventing the jawbone meltdown with implants.
All on four is a technique in which a fixed full jaw prosthesis is made on 4-6 implants on the same day. It does not cause problems in functions such as chewing, biting, taste, aesthetics, or speech. An important difference is that while 8 or 10 implants are used in traditional procedures in a completely edentulous jaw, four implants are mostly used in the all on 4 method. It can rarely be used in 6 implants. 2 implants are placed in the anterior jaw, where the ideal bone thickness is, and two are placed in the posterior jaw. The implants in the front are positioned at a right angle, and the implants in the back are positioned at an angle of 30-45 degrees. The aim here is to achieve the maximum level of durability.

Who is not made All on Four?
All on Four dental implants can be applied to many people but may not be suitable in some cases. This method is not recommended in the following situations:
- If the person has suffered severe jawbone loss, it may be difficult to place the implants correctly. In this case, other treatment options may be considered for jawbone restructuring.
- If a person has periodontal disease (inflammation of the gums), implants may not be placed correctly. Therefore, periodontal disease may need to be treated first.
- The maxillary implants are placed close to the sinus cavity. This procedure may not be performed in people with sinus problems. In this case, additional procedures, such as sinus elevation, may be required.
- Bruxism is a clenjawg or grinding disease. In people with severe bruxism, implants may be less likely to succeed. Therefore, other treatment options may be considered for these individuals.
- Implant treatment is not recommended during pregnancy. After pregnancy, the chances of successful implants may increase.
- For these reasons, suitable candidates for an "All on Four" dental implant are those with healthy gums, sufficient jawbone density, no bruxism or sinus problems, and those who are not pregnant. However, the patient must be evaluated by a dentist or dental implant specialist to make this decision.
What are the features of All on Four implant treatment in Turkey?

What are the All on Four implant types?
Acrylic or composite all on four implant prostheses:
Acrylic and composite materials are generally used to construct prostheses for all on four implant treatment. The dentist or dental prosthesis specialist decides which material to use according to the patient's needs and expectations. It is more economical than the porcelain option.
Acrylic prostheses are a common option because they are light, aesthetic, and durable. It is easier to maintain and repair. However, problems such as discoloration, scratches, and wear may occur in the long run.
Composite dentures, on the other hand, are more durable, long-lasting, and appear close to natural teeth. They are heavier than acrylic prostheses, and the construction process is long and laborious.
Porcelain all on four implant prostheses:
Porcelain All on four implant prostheses are popular, especially for patients who want an aesthetic appearance. Porcelain has a similar appearance to natural teeth and allows dentures to fit better aesthetically. In addition, porcelain prostheses are more durable and more resistant to discoloration.
Zirconium all on four implant prostheses:
Zirconium dentures are more durable than porcelain dentures and lower the risk of cracking or breaking. Also, its white color is closer to natural teeth.
PMMA all on four implant prostheses:
Polymethyl methacrylate (PMMA) dentures may be a cheaper option. However, they are less durable than porcelain or zirconium dentures.
Hybrid all on four implant prostheses:
Hybrid dentures are a combination of porcelain and zirconium. These dentures can be a durable and aesthetic option.
Metal supported all on four implant prostheses:
Metal-supported dentures can be more cost-effective than other options. However, they may have a heavier and less aesthetic appearance than other prosthetic teeth.
The prostheses are designed digitally, processed in a special laboratory environment, and handcrafted. Therefore, the manufacturing process of porcelain prostheses is longer and more costly. The weight of porcelain prostheses is higher than other prostheses. It is also more fragile than other materials. Therefore, care should be taken in using and caring for porcelain prostheses. We do not recommend this type of prosthesis if the patient does not have 6 implants. Porcelain teeth can reflect light just like real teeth. The functionality of porcelain and acrylic prostheses is the same. The difference between both prostheses is in appearance and cost.
What are the complications of All on Four procedure?
All on Four implant treatment can cause complications like any other surgical procedure. These complications are:
- Infection may occur in the area where the implants are placed. This can result in the loss or failure of implants.
- Implants must integrate with the bone for stable placement of prostheses. However, implant loss can occur if the implants fail to adapt to the bone or are damaged by infection of the surrounding tissues.
- Discomfort, cracks, or breakage of the dentures may occur if the dentures are not placed or aligned properly.
- There is a risk of damaging the nerves where the implants are placed. This can cause loss of sensation in the jaw, lips, or tongue.
- Implants can affect saliva production and cause dry mouth.
- Bleeding may occur during or after implant placement.
- Bad healing can be seen in the gums.
- Before any dental treatment, such as All on Four implant treatment, the patient should be informed about the risks and complications, and an appropriate examination should be performed before the treatment.
How to apply All on Four?
All on four implant procedure is performed in four stages;
- First inspection
- Planning and taking measurements
- Placing the implants
- Fitting the prosthesis
First inspection
Our dentists examine your mouth and teeth. If necessary, your oral x-ray is taken. During this examination, your suitability for the implant, the condition of the teeth in your mouth, and the work to be done are determined. Your teeth that need to be extracted are pulled.
Planning and taking measurements
The patient's mouth measurements are taken by our dentists using radiology and tomography methods. By measuring the patient's bone thickness, it is determined whether there is a need for bone grafting.
Placing the implants
Local anesthesia is applied to the patient for implant procedure, and general anesthesia is applied to patients with dental phobia. If the bone loss due to lost teeth does not prevent the treatment, the procedure is started. If bone grafting is required, the procedure is performed and waited until healing occurs. With anesthesia, holes are drilled with a special tool called a milling cutter in the area where implants will be placed in the lower and upper jaw, and the implants are placed in these holes. Implants are placed at an angle of 90 degrees in the anterior region of the jaw and at an angle of 30-45 degrees in the back. After the implant is placed, the temporary fixed prosthesis is attached to the implant. Thus, the patient is provided teeth during the recovery period. The implants take 2-6 months (on average, three months) to fuse with the jawbone.
Fitting the prosthesis.
The healing process is completed with the fusion of the implants to the jawbone. Permanently fixed prostheses produced by our patients are mounted on the implant after the temporary prosthesis is removed. Anesthesia is not performed during the installation of permanent prostheses because the patient does not feel any pain during this time.
